Instructions

1. You should first save this .pdf document to your computer.

A. Right click the link and choose "Save Target As..."

B. Choose a location in your computer where the file will be stored.

C. Locate the file in your computer and open it by double-clicking it.

2. After you open the file, you will see shaded rectangles on the form.

These are the areas you should fill in. If you don't see shaded areas,

click on the text button (upper right) that says "Highlight Fields" to show where you

should enter information on the form.

3. Remember to frequently save the form as you work on it.

4. After saving the form to your computer, you may close the form

at any time and come back to it later.

5. If you have questions about the information requested on the form,

6. Once you have completed your loan application:

A. print and mail it to your Preferred Home Finance Loan Officer, or

B. attach it to an email to your Preferred Home Finance Loan Officer, or

C. print and fax it to your Preferred Home Finance Loan Officer
